Southern New Hampshire Endocrinology and Joslin Diabetes Center affiliate at Southern New Hampshire Medical Center provide care for patients with diabetes and endocrinology-related conditions, including thyroid, osteoporosis, metabolic disorders, and lipid management.  Our goal is to develop partnerships in which we listen and provide the education and care that our patients need.

In addition to our Nashua location, Dr. Ellie Chuang also sees patients at the Merrimack Medical Center one day per month. Call (603) 577-5760 or (603) 429-3155 to schedule an appointment with Dr. Chuang at the Merrimack Medical Center facility.

Our practice utilizes state-of-the-art Electronic Medical Records (EMR). EMR helps improve patient care. The EMR instantly and securely puts the latest patient health information at the fingertips of our providers, whether in the office, at home, or at Southern New Hampshire Medical Center.

  • Immediate Care of Southern New Hampshire: Foundation Medical Partners and Southern New Hampshire Medical Center are proud to announce that we have expanded our Immediate Care program.  Immediate Care of Southern New Hampshire now offers care for our patients in four locations - Nashua, Merrimack, Hudson and Pelham!
